Cassia County’s Information Form for the Vulnerable Population is a form offered to families who may need assistance from Cassia County Sheriff’s Office. This form allows people to document important information about family members who are part of a vulnerable population — including people who suffer from dementia, Alzheimer’s Disease, or have an autism diagnosis.
The form will provide vital information for first responders to address the specific needs of the people on the form if there is a call for service. The information should also help first responders find the person if they are lost and return them home in a timely manner.
